Something I seriously loved from Twelve's first episode that I think has not properly paid off yet is the mirroring they did with the clockwork man. They had a long conversation mapping out the Doctor's own feelings about his continued life, his awareness of his own monstrosity, his feeling of alienation from his own face. And, again, a brutally physical episode: harvesting of people for parts, the dizzying portrayal of Clara's distress in trying to hold her breath. I remember many people being deeply repulsed by the hot air balloon made of skin, and it was somehow an exceedingly macabre moment for Doctor Who.
